• Payment is due at the time of service.  We accept Visa and MasterCard as well as cash and personal checks.

• If you have insurance, we will file your insurance claim for you.  We participate with most of the health plans in the area.  Please bring all membership cards for all the plans you are covered by.   Your portion of the visit, such as deductible or a co-pay, will be expected at the time of your visit.

• We do not quote prices over the telephone.  You must be seen in person in order to make an accurate assessment of what procedures might be indicated for you.

• A $100 fee will be charged for a cosmetic surgery consult.  This money is refunded in the form of a credit to your account if a surgical procedure is scheduled within 6 months of your initial visit.

• We see patients of all ages.   Patients under the age of 18 must be accompanied by an adult.

Friday:  8:30 a.m. - 4:00 p.m.

We prefer to see patients by appointment.  Appointments can be made during regular business hours.  We make very effort to stay on time, however emergencies and other extenuating circumstances do arise.  We will try to contact you in advance if we are unable to keep your appointment time or need to re-schedule your visit.  We ask that you extend the same courtesy to us.  If you need to cancel an appointment, please call the office at least 24 hours in advance so that we may give your time to another patient.
